Upon request, GC/MS or LC/MS/MS confirmations are guaranteed a 48-hour turn-around time. Each sample and information pertaining to the same is kept confidential. Results are disclosed to authorized personnel only; Forensic Laboratories will sign a non- disclosure agreement if necessary. Forensic Laboratories will provide toxicology services that will exceed services received from the agencies previous laboratory. Forensic Laboratories will guarantee reflected prices for 1 year. Your agency may obtain supplies at no extra charge for items such as: Requisition Forms Quantity : UA Bottles Quantity : Temperature Strips Quantity : Security Seals Quantity : Sample Bags Quantity : Shipping Supplies Quantity : Courier service for sample pick up and delivery to each collection site will be provided at no extra charge. Pre-paid postage box mailers are provided when requested. Your Agency may request on-site telephone training for completing requisition forms and chain of custody, if needed and at no extra charge. On-site training is available if requested in writing. Forensic Laboratories will analyze each sample. Samples showing presumptive positive results will be reanalyzed and then confirmed by GC/MS or LC/MS/MS upon request of the agency. The organization may customize drug tests to meet their individual needs of the agency. All specimens analyzed will have a qualitative result unless requested otherwise. Each collection site will receive their own copies of reports pertaining to their clientele. Reports are securely faxed, emailed or uploaded to a Web Portal within 24 hours of receipt. Hard copy reports are then sent by the United States Postal Service when requested. Summary reports are available for your convenience and tracking purposes. The reporting system can be customized to fit the agencies needs. Forensic Laboratories will invoice the agency on the last day of the month. The payment term for each invoice is net 30 days. Each collection site will be invoiced individually for the analysis they receive. In order to cancel this agreement, the agency must provide 30 day written notice. Forensic Laboratories will provide dedicated fax machines or equivalent to accounts that qualify.
